The Accès Infirmières team monitors test results, provides education, and quickly assesses the risk of developing complications from illnesses, while offering solutions to reverse trends and manage risk factors. Chronic illnesses can significantly reduce life expectancy and cause a loss of enjoyment if professional care is not provided in a serious and structured setting.

A person who develops one of these chronic diseases and does not control it will inevitably develop the other two.
When a person develops one of these chronic diseases—diabetes, high blood pressure, or high cholesterol—and does nothing to control it, the other two will inevitably develop and may lead to cardiovascular disease and/or disabling conditions that severely impair quality of life. These chronic diseases are insidious and can go unnoticed for long periods of time, causing irreversible damage to health.

How it works
Here we present the steps involved in the operation of our program.
Measuring blood sugar, blood pressure, and/or cholesterol levels
During the first appointment
Monitoring blood sugar, blood pressure, and/or cholesterol levels
Assessment of your risk of developing one of these diseases based on your personal situation
Assessing your risk of complications
Full or partial instruction depending on your needs
Understanding what these diseases are gives you the resources to avoid them, anticipate complications based on your risk factors, and prevent the development of any of these diseases and/or complications.

What does the program include?
- Five (5) one-hour consultations
- Four (4) 30-minute consultations
- A cholesterol test
- Blood pressure measurement (vital signs) at each appointment
- Blood sugar measurement as needed
- Calibration of the blood glucose meter or blood pressure monitor
- Follow-up phone call
Our services are offered in our clinic and satellite clinic as well as in your home. Appointments preferred.
